This form enables you to certify your taxpayer identification number and your status as a U.S. taxpayer. To comply with federal tax laws, this form, or an acceptable substitute, must be completed and signed prior to or when requesting any type of withdrawal. This is important for you because federal default withholding rules will apply if you do not provide the required certifications. And please note you will generally only need to complete this form once and then it will be kept on file for any future use.
